- Please contact your bank or cardholder immediately because on 8-27-2012 they took 126.91 from my visa card. I contacted my bank and I had to fill out a Declaration of Unauthorize/Fraudulent Use form and faxed it in yesterday, and they credit it back to me on today 8-29-2012.
