Have you ever worked for Wal-Mart?
It's not a company, it's a fund that was setup to handle the dispersion of the settlement payments.
From what I can tell this is the Qualified Settlement Fund (QSF) that is linked to a class action lawsuit against Wal-Mart in 2009 in which Wal-Mart settled.
You may have received a check during 2012 in relation to the settlement. If not, if may have gotten lost if your personal information has changed since you worked for Wal-Mart.

I'd still recommend calling your tax preparation provider (such as H&R Block) and asking them if they've heard anything about this issue. -
