TOOK OUT SHORT TERM DISABILITY INS POLICY ON 02/14/12. 1ST PREMIUM WAS TAKEN OUT OF MY PAYCHECK ON 02/21/12. HAD ONE YEAR WAITING PERIOD FOR PRE-EXISTING CONDITIONS. HAD SURGERY ON 2/19/13. WAS DENIED BECAUSE THEY SAY MY COVERAGE DID NOT START UNTIL 03/01/13. THE PAPER I AND MY EMPLOYER HAVE SAY 2/14/12 NO OTHER DATES LISTED. WAS NOT WELL INFORMED BY AGENT WHO SOLD ME THE INS ABOUT THE COVERAGE START DATE NOR THE AMOUNT OF COVERAGE I WAS TRULY ELIGIBLE FOR. HAVE SENT THEM LETTER OF APPEALS TODAY-04/12/13.
