I received this letter from Corporate Records Services, the form looks official but there's no form number or identification that ties it to any of the state or federal sites, so this was an indication that this is not a real government form instead it's a SCAM to get you to send them $125.00.
Be careful and don't send these people any money, because it is FRAUD activity.
