I did not even submit the completed form for a free sample, but I still received one. I thought it was an anomaly. Then I got a full bottle in October with a packing list listing no order information or charges and no phone number to call. I did not order the product and did not use it, but set it aside. It happened again in early November and then again today, December 12, 2013. I didn't see any charges on the card I normally use so today I checked the card I rarely use and there were three charges for $105.95. I did not authorize this company to send me this product let alone bill me such an extreme amount for it. I have returned all of the unopened bottles to them with my packing lists and copies of the card charges. I asked for a full refund and for them to cease sending me any more product.
