"I'm contacting you today regarding an order that was submitted into my office this morning. I need to verify your address to present a formal claim, either at your residence or place of employment. You have charges in the claim against you. At this point you have one opportunity to contact the firm that is handling your claim and to discuss this matter with them. They can be reached at (877) 647-2041. Claim number listed on this order is ******. You have officially been notified."
