Collection is supposed to be through an online interface, but I was led into page after page of "surveys" of advertisers, and when I thought I was nearing the end of the "required surveys" (after an hour of wading through the surveys), I was cut off after being led into a blind alley. Next thing I knew, I was back at the very beginning of the first page of surveys! So I never could reach the completion of the payment request process.
