I submitted a form to receive a free product sample after completing a short online survey. The survey presented as a popup window to the GDC website (General Dental Council UK)and appeared to be related to the website. The company used my bank details to setup a repeat monthly order-payment which i have not authorized or consented to. They claim i accepted the terms and conditions, however these were not displayed on the payment form, instead only the usual tickbox under the payment card details. Throughout the process there was no suggestion or implication of any further fees payments or continuous order. The payment form was clearly and directly intended only to the payment of the shipping expenses of the free sample (4,5$). The whole concept was exceptionally misleading. There are dozens of similar online reports from other victims.

Update: 04/08/2014

The amount is actually 236.47 not 23.647
